a) the balance used for all weighings was incorrectly calibrated 0.050 g low.
b) the mass of MnO2 was 0.081 g rather then 0.10 g.
c). the heating of the test tube was stopped before all the O2 was evolved.
d) the test tube plus MnO2 was incorrectly weighed 0.1 g higher then the actual mass, but all other masses were correct.

If everything is weighed by a balance whose calibration is wrong thne there won't be any effect to the out come percent of potassium chlorate since everything is weighed with less calibration of balance.
Case:b) the mass of MnO2 was 0.081 g rather then 0.10 g.
This should not have any considerable effect on the percent of potassium chlorate. Because we generally use MnO2 as a catalyst in the process just to speed up the process of decomposition of potassium chlorate. If at all MnO2 is added less, there won't be any effect on the amount of O2 evolved and hence the percent of potassium chlorate.
Case:c). the heating of the test tube was stopped before all the O2 was evolved
