Awareness can be created at patient level who are affected by the disease making them more vigilant and cautious of their health and treatment people with chronic kidney disease can be counsel ate regular intervals of treatment making them stress free counseling can be carried by other team members at regular intervals for designated patients. More amount of care and awareness should be created among women especially among African American as they are less are of this disease with less resources and which can be understood by given case a Africa American women suffering from diabetes and hypertension is affected with chronic disease. People with lack of resources are major populations affected by this disease so most of the awareness should be carried out in ground level moving door to door. Apart from this social media and electronic media can be used to create awareness at larger level.
