In my organization a situation that represents Care Environment Management is the cohorting of heart failure patients in an effort to improve and better manage the care of this patient population in the acute setting as well as in the discharge and transition. I currently participate on a system readmissions team made up of nursing leaders, hospitalists, specialist, and care coordination leaders to help identify ways of reducing cost of care by encouraging utilization of a heart failure pathway to reduce length of stay and reduce readmissions.From a team management perspective , I lead the CHF and COPD subcommittee where the focus is on obtaining timely referrals and follow up appointments within seven days of discharge. This remains an opportunity as we strengthen our relationships with the clinic leadership and specialists who each participate on the big system readmissions committee.
We have seen some improvement in our readmissions rates. Our current heart failure readmissions rate for our health system is 13.5% that is well below our target of 17.8% for FY2017. In addition to these efforts we are really focusing on leveraging our documentation system in an attempt to make all our referrals to our disease management clinics electronically. Currently we are still calling or emailing the clinic. The clinics must purchase our documentation tool All scripts care manager in order to make the electronic referrals. This is a focus moving forward to make the work of my team members more efficient.
What competencies should you be able to demonstrate upon completing your coursework and practicum? What could current or future employees expect from you? How could a CNL within your current setting be proactive in assessing patient or population? How could the available information management system be used to improve outcomes? Which specific outcomes? Are there opportunities to collaborate with other healthcare professionals that could be lead by a CNL who was familiar with team functions?
Upon completion of the practicum, the CNL should be able to function as a clinical and professional leader who can demonstrate the following fundamental competencies:
Advocacy
Clinical leader(support quality and outcomes management)
Team manager and Information manager(delegation, leader, educator, collaborator , communicator)
Manager of care environment ( promoting interdisciplinary collaboration, safety, and care of the patients).
Evaluator of safety and risk
In my current work environment, the CNL can be proactive in assessing our medically complex patients by way of risk stratification in an effort to help promote positive outcomes of care. Cerner is our current electronic documentation system and I think we can really leverage it more to improve outcomes by creating a unit dashboard that ties into the role of the CNL and the quality metrics they impact such as device days, sepsis, restraints etc. As leader of the Nurse Navigators this is a goal for me to create a dashboard for each of my nurse navigators. I think this adds value to the role and identifies the key areas that we impact. In regards to collaboration with other disciplines , we have several opportunities for the CNL to lead or facilitate such as: multi- disciplinary rounds, geographical rounding, etc.
Last but not least my practicum experience is busy and very interesting. I am learning a lot about other roles in the organization and I have a great preceptor!
