Emily is a 2 month old infant who was delivered by Cesarean section at 30 weeks gestation. She is the first child for Elizabeth and Creighton Broughton. Although born early, she was of substantial weight and health and was only hospitalize 1 week past her date of birth. Once home, Emily has been a "fussy" baby who cries all the time". Elizabeth has taken an extended maternity leave and stays home to care for Emily while Creighton works outside the home. Because Creighton is an executive sales manager, he frequently travels and is not able to help much with Emily’s care.
Elizabeth and Creighton bring Emily in for a 2 month follow up check at the pediatrician's office. During the nurse's family assessment, both parents appear exhausted and anxious. Elizabeth comments, "I feel like it's my fault that Emily is not getting any rest. I can’t seem to calm her down. She cries all the time. I try but I don't think I'm a very good mother. Maybe I should hire a nanny and return back to work. Creighton further states that he believes Elizabeth may be transferring her anxiety to the baby.
“I barely get in the house after traveling and working long hours and she almost throws the baby to me”. He also points out that when his mother came out to help for the first month, Elizabeth never spent any time with the baby. The couple comments that they are not sure they are doing the right things for Emily. Both state it takes a lot of time and energy to get her to sleep. Elizabeth notes she has started letting her cry for a while because she cannot seem to meet the baby’s need and besides, there is usually nothing wrong.
What assessment information (from both mom and baby) should be collected by the nurse at this 2 month check-up appointment?

Some Research shows that crying follows a developmental pattern during the first few months of life. Crying increases at 2 or 3 weeks of age, peaks between 6 and 8 weeks, and then slows down after that, generally hitting its lowest level by around 4 months. Babies also cry more often in the late afternoon and early evening, which might be when they need to release tension after a long day.
Here in Emily 's case she is two months old and it is obvious she cries alot. The nurse firstly should assess the bonding between the mother and child as the mother says she is confused what to do when baby cries and sometimes let the baby cry. Also Mr. Creighton, father of baby said that whenever he cimes from work, his wife is eagerly wsiting to give him the baby and during first month his mother took her of the e baby while Elizabeth the baby's mother never spend time with the baby. These all leads to point that baby's mother is not having much bonding with the baby and due to this she is not able to know the exact needs of the baby. This can lead to increase the crying of baby when his needs are not attended properly. For example, Elizabeth wont be knowing the reason behind the cries like whether baby is hugry, or change tge duaper, or any physical discomfort. Secondly the nurse should assess whether there is any medical issues for the baby. She should ask about the quantity of feed given, how many times they change diaper, about sleep pattern, any abnormalities seen other than crying, bowel movements etc. This is to ruke out whether it isdue to insufficient care or due to any illness.
The two nursung diagnosis can be:
1)Impaired mother - child bonding due to lack of knowledge and experience of mother regarding baby care due to first pregnancy.
2)Knowledge deficit regarding child care related to first pregnancy.
The nurse can give health education to the parents regarding:
1)importance of parent - child bonding.
2)Normal developmental stage and behavior of baby according to each age.
3) How to identify the uncomforts of baby and what all are tge reasons why baby cries. For instance following can be the reasons for the cry,
4)Also educate the parent to look for any emergency situatiin like,non stop crying with fever, crying when touched to body can indicate injury, bloated stomach, not taking feed and weight loss etc
