Goals of care change when the patient’s condition is recognized as it is terminal

The close certainty of a patient’s death is usually a very difficult situation for patients and families. It is a crucial moment for patient, his/her family and at the same time for the healthcare team to tackle. As it’s understood that at that time all the therapeutic efforts are not working and death is near. Hence in this case there is need to take proper decisions of overall care of the patient and to set a harmony with his/her family members.

Hereby all the decisions about care at the end of a person’s life emphasize on the quality-of-life offered to the patient including the promotion of comfort, relief of pain and other symptoms, and support for patient’s families, and others close to the patient.

All these actions should be conducted under the Code of Ethics for Nursing staffs and other health professionals.

In hospitalized patients whose death is close, there is need of palliative care as multidisciplinary approach utilized by specialized medical and nursing care for patients with life-limiting illnesses.

Furthermore it involves primary care including the concept of fundamental symptom management as well as the skill to sustain decision-making for patients & families, leading to relieve pain, physical stress, and mental stress of the terminal diagnosis in a patient.

Moreover these conversations about goals of care are vital when there are cases of dialysis or cardiopulmonary resuscitation as well as cancer or other life-threatening illness and when the patient is on life-extending interventions like mechanical ventilation.
