1. What plan do you suggest for handling communication problems involved in the given scenario? Why?

The plan I would suggest for handling the communication problems that were involved in the given scenario would include, effective protocol training for everyone not just Meelynn, and maybe appointing someone to her as a guide and or translator that could read and or explain the emails to her, where she could understand them. This would decrease any more patients from falling. The reason for this plan and the training would ensure that not only Meelynn but the whole staff completely understood the new patient safety program, to ensure patients would be safe from future falls. Also having someone else there to assist Meelynn such as a translator could also help by preventing future falls. The manager should always be available to assist their employees when they do not fully understand something. Patient safety should be number one, and in order to ensure their overall safety the manager needs to step up and train their staff.

Yes, the reason for this is because; when it comes to today’s healthcare systems, the delivery process can involve many interfaces and patient handoffs among the many health care professionals with varying levels of educational and occupational training. For example, during the course of a 4-day hospital stay, a patient may interact with 50 or more different employees they include; the physicians, nurses, technicians, and other staff members. Effective clinical practice then involves many instances where critical information must be accurately communicated. Team collaboration is very essential. When health care professionals are not communicating effectively a patient’s safety is at risk for many reasons such as; the lack of critical information, misinterpretation of information, unclear orders over the telephone, and overlooked changes in status. Lack of communication creates situations where medical errors can occur. These errors have the potential to cause severe injury or unexpected patient death. Medical errors, especially those caused by a failure to communicate, are a pervasive problem in today’s health care organizations. According to the Joint Commission, “if medical errors appeared on the National Center for Health Statistic’s list of the top 10 causes of death in the United States, they would rank number 5—ahead of accidents, diabetes, and Alzheimer’s disease, as well as AIDS, breast cancer, and gunshot wounds.” The 1999 Institute of Medicine (IOM) reports, “To Err Is Human: Building a Safer Health System, revealed that between 44,000 and 98,000 people die every year in U.S. hospitals because of medical errors. Communication failures are the leading root cause of the sentinel events reported to the Joint Commission from 1995 to 2004.” the Joint Commission also cites that communication failures as the leading root cause for medication errors, delays in treatment, and wrong-site surgeries, as well as the second most frequently cited root cause for operative and postoperative events and fatal falls. For written communication:

1. Relevance; this can include having too little or too much information. For example, too much information that is included can be a waste of time and efforts on the part of the writer, but this can also distract the attention from the actual central message that is trying to be conveyed. Providing more information that what is necessary is most likely to hamper communication than enhance it.

Too little information that is omitted or glossed over, the end result may be less than satisfactory. Written communication has to ensure that the essential points are conveyed if the process is to be an effective one.

2. Clarity; to prevent misunderstanding, it is essential to make clarity of expression a major priority. The basic rule here is to ensure that you express yourself clearly.

Two major problems with verbal communication within an organization include:

1. Cultural; People from different cultures often have difficulty communicating. For example, if two people do not speak the same language; they will need an interpreter. Even with interpretation, communication barriers can still exist. Different cultures have disparate systems of idiom and etiquette, this in which can cause major problems. For example, if one person uses phrases that do not translate easily into another language, the full intent of the message can be lost.

2. Confused messages; even if a person does their best understanding what someone is saying they still miss the main points. If you express yourself as clearly as possible, the other person might miss yours. In order to avoid this is to repeat back the other persons concerns to ensure that you understand everything correctly.

The actions that an individual can take to ensure that their communication is effectively received include. No matter what type of business and or organization is being operated, effective communication can prove being beneficial. Communication can take the form of written, face-to-face or group interaction, and it is vital to the success of small businesses and or large organizations such as health care. You can implement a variety of techniques to ensure the success of communications. Developing basic skills are important; for example, effective communication can begin with mastering basic skills such as listening, speaking, asking questions, and providing feedback. As a leader, by practicing and implementing these skills on a regular basis, the employees will feel that the communication channels are open and that they have input into their job roles as well as the direction for the organization. Others include; giving feedback, monthly meetings, clarity and brevity, and the use of technology. This is very important because, Effective communication can reduce confusion among your work force and keep everybody on the same page.
