Question

QUESTION 1

The hospital HappyChild’s Emergency Room (ER) in the community of Sunnyland recorded 120 asthma cases during 2005 out of a total

of 1450 individual children seen.

The state Children with Special Health Care Needs (CSHCN) program was interested in determining if these numbers meant an increased risk of a child developing asthma in Sunnyland or a possible decrease in the effectiveness of the medication taken by asthmatic children in this community . In 2005 they asked the hospital to track the number of first time asthma diagnoses admitted to the ER versus those asthma cases who visited the ER but had been previously diagnosed as having asthma.

In 2006 out of 1370 children seen in the ER, there were 130 asthma cases: 42 were new cases, 88 had been previously diagnosed prior to the ER visit.

1. What was the risk of having asthma in 2005 for children in Sunnyland seeking ER care at hospital HappyChild? (20 pts)

2. What was the prevalence of childhood asthma in 2005 in the ER at hospital HappyChild in Sunnyland? (20 pts) 120

3. What was the prevalence of childhood asthma in 2006 in the ER at hospital HappyChild in Sunnyland?   (20 pts)

4. Can the state CSHCN program answer its question? ( 15 pts)

5. Does knowing the incidence of childhood asthma diagnoses in the ER at this hospital in Sunnyland in 2006 help? What is the incidence? ( 15 pts)

6. What else is needed to answer the CSHCN program’s question? ( 10 pts)

Explanation / Answer

1. 2005 EMERGENCY ROOM ASTHMA CASES RECORDED= 120

TOTAL CHILDREN CASES = 1450

IN 2006 ASTHMA CASES 130

TOTAL CHILDREN CASES = 1370

NEW CASES IN 2006= 42

OLD CASES = 88

1) RISK OF ASTHMA IN 2005 CAN BE CALCULATED AS FOLLOWS=

NO OF DISEASE ONSET/NO. INITIALLY AT RISK

= 120/1450-120= 120/1330= 0.09022 = 9%

2)PREVALENCE OF ASTHMA IN 2005

PREVALENCE RATE= ALL THE ASTHMA CASES AT THAT TIME PERIOD(NEW+OLD)/TOTAL

= ASTHMA CASES RECORDED IN ER/ TOTAL CHILDREN VISITED

= 120/1450 = 0.0827

= 8.27%

3)PREVALANCE OF ASTHMA IN 2006

PREVALENCE RATE= ALL THE ASTHMA CASES AT THAT TIME PERIOD(NEW+OLD)/TOTAL

= 42+88/ 1370

= 130/ 1370= 0.094= 9.4%

4)YES UPTO SOME EXTENT , THEY ARE ABLE TO ANSWER THEIR QUESTIONS.

but the children who visited ER in 2005 who had previouslyvisited ER back in 2004 is not given, but the information is there for year 2006. So it will help in some extent.

5) Knowing the incidence of asthma in 2006 still help to some extent but to get a robust data and interpretation one needs to know the incidence for other previous years as well (previous 10-20 years).

6) The incidence in previous year in 2005 and years back to 2005 , atleast 15-20 years data is taken into consideration to reach a robust conclusion of asthmatic incidences
