a) The incident refers to "The Domino Theory".
According to this theory 88% of all accidents are caused by unsafe acts of people, 10% by unsafe actions, and 2% by act of God.
This theory was given by W. H. Heinrich in year 1931.
He also proposed 5 factor accident sequence as under-
1) ancestry and social environment
2) worker fault
3) unsafe act together with mechanical and physical hazards
4) accident
5) damage or injury
Heinrich proposed that to prevent the accident removal of one of the factor is required with 3rd being the key.
Thus in our caseprevention of accident can be done by removing and analysing theb 3rd factor ,i.e. unsafe act of workers.
