Anytime there is relative motion toward or away from the the source of the wave there will be a Doppler Effect.
The first three cases...
A police car is moving away from you while traveling 40 mph. You are stationary.
A fire truck is stationary. You are moving away from it while traveling 40 mph.
An ambulance is moving towards you while traveling 50 mph. You are moving away from it while traveling 40 mph.
Will all produce a Doppler Effect
The other two cases, will not produce a Doppler effect since there is no realtive motion that causes a difference in linear distance between you and the source.
