Question

Five years ago, a patient presented again to her PCP, this time complaining of a productive cough and stiffness and pain in her hands and feet that seemed to come and go to affect different joints. she is afraid that she is developing rheumatoid arthritis like her older sister. Her BP at this time was 140/90, HR 105 and she has a temperature of 100 degrees F. Auscultation of the lungs revealed abnormal lung sounds, suggesting that she had bronchitis. A chest x-ray revealed mild pulmonary edema but no white blood cell infiltrates in the terminal airways. The PCP was concerned about susceptibility for developing pneumonia. Axillary and inguinal lymph nodes were slightly enlarged. blood tests revelaed an HCT of 43%, a PLT count of 330,000mm^3 and a total WBCcount of 1,200/mm^3. A urinalysis was essentially normal. which is the three blood tests results directly above would be of most concern? Give a likely cause for the abnormality. The patients WBC differential was : 75% neutrophils, 15% lymphocytes, 5% monocytes/macrophages, 4% eosinophils, and 1 % basophils. which one of these 5 white blood cell types has been specifically targeted by the patients immune system?

Explanation / Answer

Three blood tests which can be done to diagnose pneumonia are,
1.Hematocrit- red blood count.
2.CBC (Complete blood count)-white blood count(Differntial count)
3.Blood culture-to identify the bacteria.
* cause for the abnormality may be malnutrition, any illness or may be because of oldage.
*lymphocytes are specifically targeted by the patient immune system.
