Question

1.Answer the following in terms of chronic inflammation. List 3 distinct conditions that result in chronic inflammatory that are not infectious in nature and describe how this leads to an increase in cancer risk.
2.A tumor can be described as a wound that will not heal. Explain in as much detail as you can what is meant by this statement. Compare and contrast wound healing and tumor formation. Make certain to mention any important cell types and their functions.

Explanation / Answer

Three chronic inflammatory conditions which are not infectious :

1- Bronchitis

2-Pancreatitis

3- Rheumatoid arthritis

How chronic inflammation leads to cancer ?

1- Release of factors that promote proliferation : Infiltrating leukocytes and activated stromal cells have been shown to secrete a wide variety of growth factors , such as EGF and proteases that can liberate growth factors from ECM.

2- Removal of growth suppressors - Cancer cell growth is is suppressed by cell-cell and cell- ECM interactions . Proteases released by the inflammation cells can degrade the adhesion molecules that mediate these interactions.

3- Enhanced resistance to cell death - Tumor associated macrophages may prevent anoikis by expressing adhesion molecules such as integrins that promote direct physical interaction with tumor cells .

4- Inducing angiogenesis by factors like VEGF which can simulate angiogenesis.

5- Evading immune reaction including TGF beta like immunomodulators.

Second question -

Cancer mimics wound healing in a sense that :

1- Formation of new blood vessels ( angiogenesis ) and increased vascular.permeability.

2- Arrangemnet of moleculer matrix around the cells.

3- Changes in cellular attachment.

While all this result in wound healing , this all leads to the progression or maintainence of tumor . Thats why a tumir is a wound that will never heal.
