9. The first question should have been given two more options such as "Probably Yes" and "Probably No". Moreover, the question could have been reworded as "Do you feel that poverty and lack of supervision can contribute ....."

The second question, should also have an option of "None".

The third question is vague. Should be specific about what responsibilities instead of just reading story to your child. Also, should have two more answer options such as "Almost everyday" and "Sometimes"

Fourth question and answer options is strong and nicely designed.

Fifth question should have been two different questions. This is because, if the reader sees that they need to describe the event, they might be tempted to lie and hide that they were assaulted sexually. When separately asked, atleast we can get the accurate count of 'Yes' and 'No' of sexually assaulted victims, even when they do not want to specify in detail about that bad experience they had.

Sixth question should have been reworded as "...volunteer OR donate" instead of "...volunteer AND donate"

Seventh Question should have been split into 3 following questions:

Eighth question - well worded and nicely designed.
