a) Here the patient has low plasma sodium concentration and lower than normal urine osmolarity. This occurs in conditions like primary polydipsia and malnutrition in which the osmo receptors are reset producing dilute urine.

b) Here, the plasma concentration of sodium is low but high in the urine. This occurs because of the impaired ability of the kidneys to dilute the urine. This may occur due to increase in the levels of anti diuretic hormone.
