A) Initially we can think about the hypothyroidism which means decreased secretion of thyroid hormone. If we look at the symptoms of the patient which are weight gain, feeling cold and loss of energy or fatigue all these are symptoms of hypothyroidism . The functions of the thyroid hormones are to increase metabolic rates and to maintain body temperature.Now you can have the idea that due to impairment of the hormone there are chances to have all these symptoms. B) If we look at the lab report then T3 level is normal, T4 is less and increased amount of TSH. If there is low level of T4 , pituitary gland will release more TSH which will cause elevated level of TSH and if TSH level increases then conversion of T4 to T3 increases and thats why your T3 level is maintained but due to decreased level of hormonal secretion T4 level is less.So, this is the clear case of primary hypothyroidism C) As we have already discussed the symptoms of hypothyroidism are fatigue,cold intolerance,face swelling,weight gain,depressed hair growth and the lab report also suggests that its a case of hypothyroidism.Most importantly hypothyroidism leads to myxedema in adults and cretinism in children.
