Case Study #1

A woman complains of severe weakness and heart palpitations, after a night of vomiting and diarrhea from an intestinal flu. An ECG revealed mild cardiac arrhythmia and changes in the normal ECG tracing. Her breathing rate is low and her muscles weak. Analysis of plasma ion concentrations resulted in the following:

Na+ = 138 mEq/L, K+ = 1.8 mEq/L, Ca2+ = 4.7 mEq/L, Cl- = 101 mEq/L.

In the space provided below, please define and describe each disorder. Circle the disorder you believe is the cause of the described symptoms. Explain your choice, including why the other disorders were eliminated.

Answers :-

1. Disorder causing the given symptoms is - hypokalemia.

That is lower amount of potassium in blood its given 1.8 mEq/L where the normal range is 3.5 - 5 mEq/L.

As, hypokalemia causes the interference in electrophysiology of heart where it leads to decrease in electrical conductivity in heart and increase the threshold, action potential duration & potential of resting membrane.

1. Hypernatremia - its a disorder where the Na+ ion goes above the normal range that is 135 - 145 mEq/L

2. Hypokalemia - its a disorder where the K+ ions goes below the normal range that is 3.5 - 5 mEq /L

3. Hyperchloremia - its a disorder where the Cl- ion goes above the normal range that is - 96 - 106 mEq/L

4. Hypocalcemia - its a disorder where the Ca+ ion goes below the normal range that is - 4.7 - 10.2 mEq/L
