4. Rh + means that blood has Rh Antigen present and in Rh - patient there are no Rh Antigen.

5. In patient having O - blood group there are NO antigen present therefore there will not be any new Antibody formation in patient receiving blood of O - . Therefore he is Universal donor.

But O - patient has following Antibody present - Anti A, Anti B and Anti Rh therefore if any other blood (for example Blood A type is give then Anti A will react with Antigen A which are present on donors blood and will lead to hemolysis) is given then antigen-antibody reaction will occur and this will lead to hemolysis and transfusion faliure.
