She mentioned feeling as though she has been "on an emotional roller coaster" during the past year, throughout her on-again/off-again relationship with a 35-year-old married man. The last breakup with him seemed final, and Marcie has felt herself sinking deeper and deeper into depression ever since. When probed further about suicidal ideations, Marcie admitted that she has considered killing herself, although she is uncertain whether or not she would actually do it. Reflection Questions: 1. 1. Does the primary diagnosis via the current DSM play a factor in what you would do as an administrator of the facility?

The Diagnostic and Statistic Manual (DSM) in the current setting focuses on the various axes for evaluation. In the case of Marcie, the client - The symptoms related to Axis I defined here are lack of interest in general and sleep disorder. Symptoms on Axis II that are due to unchecked Axis I symptoms include the feeling of being on an 'emotional roller coaster' because of a certain relationship. Marcie's evaluation does not cover any parameters under Axis III except for the sleep disorder. A factor in Axis IV has been the last breakup in the said relationship which has made Marcie feel worse over time. With these factors in play, the Axis V of DSM should state that: It is safe to say that as an administrator of the facility proper care system should be put in place ensuring that Marcie is cared for, her concerns are addressed and therapy is started with proper consent. The aim of therapy should be to ensure that Marcie does not feel threatened or exposed and that she is able to undergo treatment at her own place.

  1. The patient should be under proper security such that they do not wander off.
  2. The patient should not have the possibility of having a 'safe place' away from the sight of the healthcare professional.
  3. If the patient is particularly prone to violence, the restraints used should be checked periodically.
  4. The patient should not have access to anything that may put them in harm's way.
  5. The patient should not have the option to abuse drugs or medicines prescribed to them.
  6. The environment that the patient is put in should be comforting and not be threatening. This should be implemented by proper training of the support staff in the community or mental health care facility.
  7. Simple changes to the premises such as wall fans or use of plastic cutlery can reduce the risks of a patient putting themselves in harm's way.
