Situation : silhouette analysis of human body

Aim : To collect physical measurements of a person and suggest clothes, diet etc. based on their body size

I would use two input tags to collect information about the person such as name and contact details.

Check box tag for selecting many applicable features such as thin legs, fat knees etc.

Radio tag for selecting one out of many options. ex: broad shoulder, narrow shoulder, average shoulder etc

Select tag for date of birth

text area tag for their specific body data which can be accomodated in the steps of analysis.

submit button to submit data to the server

reset button to reset the form
