In ER -model, Entity Instance is nothing but a single occurance of an entity type.Based on Entity instance we can provide cordinality between two entities. Usually cardinality specifies that how many instances of one entity relate to another entity's instance.
As per given ER-diagram CareProvider is an entity in healthcare database ER model.
Careprovider entity having instance cordinality with insurence plan and person.
There is only one insurance plan for one carprovider.
Another one is person, one careprovider may treat or check so many people.
