Case History:   This 37-year-old married African-American male is admitted to the orthopedic service of the hospital. He complains of pain when walking and golfing. He says that his knees have been “painful” for many years since he quit playing semiprofessional football, but the pain has become very severe in the last six months. He is scheduled for arthroscope. His preoperative diagnosis is degenerative arthritis, left knee, with possible tear, medial meniscus.

Operative Report: After induction of endotracheal analgesia, patient was positioned on the operating table and a tourniquet applied over the upper left thigh. Following positioning of the arm in a circumferential holder, the end of the table was flexed to allow the leg to hang freely. The patient’s left leg was prepped and draped in the usual manner. Following inflation of the tourniquet to deprive the knee of blood flow, the tourniket was inflated to 300 mm Hg. The knee was inspected using anterolateral and anteromedial port holes.

Findings: The synovium in the suprapatellar area showed moderate to severe inflammatory changes with villi formation and hyperemia. The undersurface of the patella showed loss of articular cartilage on the lateral patellar surface with exposed bone in that area and moderate to severe chondromalacia of the medial surface. Similar changes were noted in the intercondylar groove. In the medial compartment, the patient had smooth articular cartilage on the femur and moderate chondromalacia of the surface of the tibia. The lateral meniscus appeared normal with no evidence of tears and a smooth articular surface on the femeral condyle. The examination and probing were repeated with no additional pathology being identified. The tourniquet was then released and the ankle flushed until the bleeding was slowed. The wounds were Steri-stripped closed, a sterile bandage with an external wrap applied, and after surgery, the patient was returned to the preoperative recovery room in stable condition.
