Case 1 Homework Mr. and Mrs. Fike had a normal daughter 2 years after they were married. Two years later they had a son and named him Trevor. He weighed 7.8 pounds at birth and also seemed to be perfectly normal. At 3 months of age, Trevor developed a runny nose and a persistent dry cough. One month later he had a middle ear infection (otitis media) and his pediatrician treated him with amoxicillin. At 5 months of age Trevor had a recurrence of otitis media. His cough persisted and a radiological examination of his chest revealed the presence of pneumonia in both lungs. He was treated with another antibiotic, clarithromycin. Mrs Fike noticed that Trevor had thrush (Candida spp.) i rS. in his mouth (see image below) and an red rash in the diaper area.

Explanation / Answer

1. Otitis media is the inflammation of the middle ear characterized by the accumulation of infected fluid in the middle ear, resulting in bulging of the ear drum, and pain. It is the most frequent disease occur in children.

Oral thrush occurs when your immune system can’t defend against the Candida albicans fungus. As the children have weaker immune system, they cannot fight with diseases like Candida albicans. This results in the occurance of oral thrush.

2. Oral polio vaccine and DPT vaccine were given to the child to avoid the occurance of diseases like polio, diphtheria, pertussis, and tetanus. The paediatrician had given him these vaccines because he observed that the child was not gaining weight.
