Since drinking the grapefruit juice and taking cholesterol-lowering medications, her cholesterol level has started to see a rise although her diet has not changed. A known effect of the juice is to inhibit the effects of the medications. How would you determine that Mrs. Jones is drinking grapefruit juice. What questions would you ask to determine if Mrs. Jones might be having potential side effects? Mrs. Jones asks if she can take her medication two hours after drinking her juice because she thinks the grapefruit helps her arthritis. How would you respond? Grapefruit juice decreases the activity of the cytochrome P450 3A4 (CYP3A4) enzymes that are responsible for breaking down many drugs and toxins. Grapefruit contains compounds known as furanocoumarins that block the CYP3A4 enzymes. When grapefruit juice is consumed, the enzyme’s ability to break down the drug for elimination is decreased. Blood levels of the drug may rise, resulting in the risk for new or worsened side effects

Grapefruit also interacts with drugs used to treat nausea and urinary tract infections, immunosuppressants, medications to treat cancer and many drugs that affect the central nervous system, including anti-anxiety drugs.

Grape fruit worsened the side- effects of medication. Dr. should ask whether she is feeling pain in muscles, whether she has problem .

In digestion or have some rash on her body she is feeling nausea.

Taking medications at a different time from when grapefruit juice is consumed will not prevent the interaction. The effects of grapefruit juice on certain medications can last for over 24 hours.
