Which study design was used to conduct this study? Be specific in describing the study design (ie prospective cohort, experimental, case-control, etc). A local health department has received a grant to study the community factors that are important in maintaining a healthy weight in the population. Four communities are recruited and mail questionnaires are sent to all residents. The survey ask questions about nutrition, physical activity, height, weight, and locations where physical activity occurs. Responses are tabulated for each community to develop a description of factors that seem important.

The study design used to conduct this study was retrospective cohort, because in this study the past event of four different community were collected to analyze the health status of those community. All those study in which the collection of data of past events (before exposure to a disease or for making plans to not suffered from disease) is carried out came under the retrospective cohort study design.
