Competencies include those of the CNL role function: effective and ongoing communication with the multidisciplinary team while advocating for the healthcare of patients; improving patient care outcomes and patient satisfaction; minimizing cost and reducing waste; educating staff on unit-specific evidence base practice; and being a change agent at the point of patient care.

Current and future employees should expect a CNL to demonstrate and exhibit leadership while effectively and professionally communicating and educating staff, understands the CNL as an advanced generalist in education of general medicine, is well organized in the pursuit of designing and providing quality patient care outcomes. and be able to delegate and supervise to provide quality, safe patient care. By understanding the healthcare system in which the CNL works, the CNL understands the importance of communication and respect needed at all levels and the need to improve systems to enhance outcomes. All departments are invested stakeholders in provision of care; no department should work in a silo.

As a CNL is my hospital, I understand the importance of working with all departments and multidisciplinary team members in enhancing communication, collaboration, facilitation and implementation of care processes that would improve patient care outcomes. Healing and healthy lifestyles depend on nutritional status. Our dieticians need information, i.e., height, weight, documentation of percentage of meals eaten properly documented in the electronic health record. Nursing as well as CNAs are not documenting this important information. The CNL, while working with dieticians, has the unique opportunity of educating her staff on documenting. Other departments the CNL has the opportunity to collaborate and streamline processes with are Respiratory Department and Pharmacy. When placing orders in our current system, if a medication and/or treatment needs to be changed; i.e., metoprolol 25mg to metoprolol 12.5mg, there is an item that says ‘change’ or ‘discontinue.’ Unfortunately, if the operator chooses ‘change’ pharmacy must then discontinue and reorder the drug and/or treatment. Most staff are unaware of this problem. This has caused multiple errors. Staff are continuing to handwrite over the medication reconciliation form faxed to pharmacy. This has caused multiple errors. Nursing also needs to be educated on treatments and side effects.

Unfortunately, my hospital allows physicians to write orders and progress notes. Our current system is sunsetting, and we will obtain Cerner next year. In the meantime, issues and problems we have faced and continue to face are causing problems for departments, and errors have been occurring that affect patient outcomes. The CNL, working with the informatics specialist, dieticians, pharmacists, and respiratory therapists, would be instrumental in collaborating to find important resolutions to improve patient outcomes.

I spend my days now with the Quality Improvement Coordinator and work towards educating staff on proper documentation of medication histories. She is a wealth of information, and has proved an excellent resource. I have met with the Director of Education and Informatics, along with the Quality Improvement Coordinator, in regard to medication history documentation and utilization of Dr First.

Clinical Nurse Leader is the person who manages the healthcare by leading the nursing staff and managing the team in order to provide best quality service for the patients. Teamwork is the key factor for any organization and it's the responsibility of leader to motivate exiting as well as new staff continously in order to provide quality service to patients. CNL maintains the positive communication and co-ordination with all departements of healthcare so that she or he can lead the team from the front.In order to maintain quality service updating the exiting system to the available new resources is important either it's EHR system. EHR is helpful in maintaing patient data and proper documentation regarding patient data ,history, patient treatment information, prescribed drugs etc. Such data's are very helpful in follow up treatment.
