Question

5) Assume 3 patients are admitted to the ER at the same time. Each patient is given an IV to quickly administer 2 liters of fluid. The fluid each patient receives is: Adrienne receives 100% water Bart receives 0.9% saline Catherine receives 5% saline A. One or more of these patients will experience problems after fluid administration. Name those patient(s) and describe what problems each will experience and why. Include a discussion of how cell structure and function are affected by the fluid administered. t of tonicity used in clinical practice. Provide a specific example.

Explanation / Answer

A) The 100% water is hypotonic and 5% saline is hypertonic to plasma. But, 0.9% saline is isotonic to plasma. So, Bart will be unaffected while both Adrienne and Catherine will be affected. In Adrienne, the cells increase in size due to osmotic effect of hypotonic fluid and in Catherine the cell shrinks because of the same reason.

B) The concept of tonicity is made use of in fluid administration. For example, during dehydration or burns injury fluid that is isotonic with plasma are administered to compensate for the fluid loss.
