It can be based on a current or former employer, a local company, a company that you would like to work for someday, a community organization, etc. This assignment will be more difficult if you choose a current employer because it could cause problems. (When your respondents fill out your questionnaire, you can tell them what their roles are - customer or coworker or management or other.)
2.) Tell me what your position/role with that company/organization is (for the purpose of the proposal).
3.) Tell me what issue/concern/problem you envision as the focus of your proposal.
4.) Tell me who will be the target audience of your questionnaire …Co-workers? Customers? Investors? Suppliers? Etc.? Your target audience of the questionnaire is not the same as the intended recipient(s) of your proposal which will most likely go to a CEO, business manager, doctors, president, board of directors, etc.

Dear Sir,
I am, Jenny Davis, a senior associate of Human Resource of Accenture India, Pune branch. Since our company is on drive to measure the employee satisfaction levels, we have decided to heck different parameters of employees from the satisfaction point of view. One of the point is work life balance questionnaire.
As you are ware sir that, finance though not a main stream branch like IT, but is of great help in our organisation. Hence we should also consider their view points. Employees of finance dept have to sit back for longer hours, so we would like them to fill out this work life balance. This would be of great help as to decide any strategy, any shift, or to let us know any problems faced by the employees.
