A chronic alcoholic is brought into the Emergency Department vomiting and complaining of abdominal pain. Overall this patient looks really sick. The blood panel and ABG come back as follows:
pH = 7.40 Na+ = 137 Urines
Paco2 = 41 K+ = 3.8 Ketones +3
Pao2 = 85 Cl- = 90 very yellow
HCO3- = 22 HCO3- = 22
Glu = 138
BUN = 20
List at least two things that might interfere with this patient’s overall wellbeing. Explain your listings based on physiology
Explanation / Answer
The condition is Hypochloremia.
The chloride ions are also lower than the normal range(97-107mEq/L)
The presence of more ketone bodies in urine also gives the yellow color to urine. blood glucose is in the normal range. more loss of sodium and chloride in urine also impart yellow to urine.
Here the sodium levels also little decreased in hypochloremia. this may lead to abdominal pain and vomiting.
