Prepare a formal complaint appropriate to the reader – the company or individual who is responsible for the issue. The business you are writing to must be a real business in which you can provide an address. Be specific in who you are addressing the letter to.

I am John Evans and I bought a Samsung galaxy s6 edge plus (SMG928V) on 23rd September 2018 and was disappointed to discover that the phone has the following problems:
The [Consumer Rights Act 2015/ Sale of Goods Act 1979] makes it implied that the contract of the product should be as described and has standard quality and it is fit for use without causing any damage to the consumer.
As it is discovered the phone does not have the standard quality and that it might cause damage/ injuries and that it poses a threat to my life I would Kindly request you to replace the phone with a new phone or refund my money.
