Patients sought care from physicians and accepted their opinions and treatment choices without question. However, in today's healthcare industry, there has been a shift away from paternalism as patients become active in their treatment choices and work as a team with their healthcare professionals. In the light of these facts, answer the following questions: What do you feel has caused this shift and why? Do you think this shift is for the better? How do you think this team approach to care can affect a patient's care choices? Scenario 2 With increasing diversity in the United States, a question has arisen regarding what accommodations should be made to assist the diverse population. Some groups or individuals feel in order to address the increasingly diverse population; we must employ a medical staff that is reflective of a community's population. For example, if half of the population in a community is Hispanic, the medical office staff should be comprised of Hispanic workers. Do you agree or disagree with this solution for addressing diversity in healthcare and why or why not? How would you address these diversity issues of the Unites States? What do you feel has caused this shift and why?

In earlier days, patient’s has the assumption that doctors knows best and they use to follow whatever doctor recommends. But, now a day you can see a change in the trend, where patients more educated and are aware about different clinical conditions and are interested to take much closer look in their health. This is good scenario, when the patient is aware about his condition and the way of treatment he is undergoing, there is very less chances of any error might be happening (1)

Do you think this shift is for the better?

Patients have the right to know what is done to their own body during the treatment and they have the freedom to decide the way of treatment. In some aspects it is good, but in some cases it has been observed that the patients wants to include some changes in the treatment plan and in some occasions it is not possible for the clinicians to listen to them completely and follow the plan as per their instruction. It is the clinician who knows better about the patient’s condition and the treatment methods. In some cases this approach may be appropriate, but in some cases it is not, at that time the physician may decline the proposals of the patient if they are in appropriate and going to affect the treatment process (2).

How do you think this team approach to care can affect a patient's care choices?

In earlier days, all the decision regarding the treatment was taken by the physician as he is the only person with full knowledge about the clinical conditions. But over the time the situation has changed, now we can see along with physicians other technicians and trained staff also work in the diagnosis and treatment of patients. Now not only physician, but the reports obtained out of the diagnostic labs also gives valuable information and this gives the patient more freedom to know about his clinical condition and different treatments available (3).

The diversity of United States is increasing at a continuous rate and patients coming to hospital are having language and cultural barriers. To provide better treatment, healthcare professional must be able to interact with the patient about his present and past condition.

Yes, I agree with the proposal of some groups or individuals, who feel to address the increasingly diverse population, hospital must employ a medical staff that is reflective of a community's population.

To address these diversity issues of the Unites States, we should ask hospitals to prioritize the inclusion of diversity in their recruitment or hiring process, so that they can incorporate cultural competence among the workforce (4, 5).

Among the main disparities found by the United States healthcare system, it is difficult to completely solve race, gender and socioeconomic disparity, but we can work towards resolving disparity regarding the socioeconomic. The access for better treatment for the poor can be increased by offering health plans or incentives and can consider the ways in which they can pay the hospital bills.

Hospital should provide physical environment which can support the patient’s diversity and should give space for the open discussion about their problems.

Hospital can try to take care of cultural diversity by looking at the requirement of prayer rooms or family conferences.

Some departments like gynecology and obstetrics need female staff to address female patients, recruiting female staff in these departments can increase the patient in flow (6).
