1) EHR - Electronic Health Record
EHRs (Electronic Health Record ) is digitalized version of patient's normal paper chart. These are real time records which makes information avaialbility instantly and very securely for authorized users. It can contain:
a) patients's completes medical history including diagnoses, treatment, medications, test images, laboratory results, surgical records etc.
b) acces to the tools that is used by doctors or providers to make decision about patients's health
c) Streamline and automated workflow for the providers.
2) Interoperability
Interoperability refers to the computer systems ability to communicate and connect among one another readily, irrespective of their different manufacturers for different industries. But Interoperability in EHRs is the biggest challenge for healthcare industry. The rate of Interoperability achived by EHR softwares are very low. By 2015, only 6% of healthcare providers could share patient data with other clinicians who use an EHR system different from their own. This significantly limits the efficiency that EHR systems were supposed to offer.
Importance of Interoperability :
Improves efficiency: When the data is shared no matter what the source, its easiers for the providers to get into the matter and take decisions about the treatment
Reduce treatment cost: Data like blood test report can help doctors to know about patients blood type and any disease related to the same like Diabetis etc. and there is no need of doing the test again during the emergencies.
Transitions of care: It enables safer transitions of care which leads to better patients outcome overall. For example, if a patient on vacation falls ill, he may not be able to provide his details about medical history which can make difference to doctor taking his care. But with the systems, it is very easy and reliable.
Advantages of EHRs:
a ) It improves patient care. Specifically in Small heath care it is very beneficial as they have limited resources.
b) It increases patient participation.
c) It improves care coordination among different providers.
d) It improves diagnostics and patient outcomes during various emergency conditions
e) It will improve effiecicy and very cost saving for patients as well as providrs
Challenges using EHRS:
a) Interoperability is one the biggest challenges in using EHRs. It requires the collaboration of various stakeholders, including patients, providers, software vendors, legislators, and health information technology (IT) professionals. data have become more of a commodity and competitive advantage than a basis for coordinated care. Vednors and providers do interfer with the information between the EHR systems to earn money and make their prominent dominance.
b) Security - Security of patients data is one of the biggest task in itself. EHR collects a huge amount of data and there is always cyber threat hangs with the source.
c) Clinical burnout is another challenge in using EHRs which make prodiuctivity of staff go down and possible issues with patient safety make the way in.
Clinical burnout may not be the issues in small clinics but Hospitals or acute care where different staff has different responsibilities and need coordination.
