Please help me with this scheme

As the Secretary and PCI chair of the NSBE chapter of Wentworth, we decide to create a database of the NSBE Wentworth chapter members and their contribution to the club, which will include attendance, payed dues, etc. We think for our application, we can use this database to help the NSBE Wentworth chapter’s Executive board track the members progress which will be used later to evaluate toward future position elections and conference price reduction. We will create an application database from SQL that will help the Executive board search up members or any other attribute they desire, and see members’ results.

I need someone to help me to create a table in either sqlight or sql. I only need the headers of the tables, to look like the scheme above. I'm being very specific, I only need this, I dont need info in the tables yet, just the first row fill.

column name you can add or delete as per your requirements.this is an example.

insert 1 row :

INSERT INTO nsbetable VALUES

('124','raseena','9656377244','my address is this','2016-04-01','yes','2000','my work isthis');

   this is how you can insert values into table.
