Question

A 33-year-old man comes into the hospital for a checkup. For the most part, the physician sees that everything is alright with the patient, but the patient complains about pain in the perianal region. The doctor rechecks the results of the patient and realizes that he lost about 30 lb. since the last time he came. When asked, the patient said that he has not been doing anything different in terms of lifestyle, but he has been enjoying a new BBQ place that opened 14 weeks ago. What other tests should be performed or what is your diagnosis?

Explanation / Answer

The doctor should perform a digital rectal examination to check for anal fissures. Flexible sigmoidoscopy and colonoscopy can be done to confirm the diagnosis. Anal fissures is a condition in which there are small tears in the perianal area which pains more on spicy food like BBQ and other food items which make the stools hard.
