Explanation / Answer
The steps of nursing process are as follows:
1. Assessment Phase- Information about a patient's psychological and physiological as well as sociological status is gathered by a nurse. The data is collected through interview, physical examination, patient's health history review and family history of the patient. For assessing data general observation is also done.
2. Diagnosing Phase- An educated judgment is made by the nurse regarding actual or potential health problem with the patient. Sometimes for a single patient multiple diagnoses can be made. The assessments include an actual description of the problem and can also mention patient's risk to develop further problems. Diagnosis phase is critical in determining the treatment path for the patient.
3. Planning Phase- Once the diagnosis is agreed upon by nurse and patient development of plan of action takes place. In case of multiple diagnosis each assessment is prioritized by the head nurse. High risk factors are given special attention. Measurable goal for beneficial outcome are assigned to each problem. Evidence-based Nursing Outcome Classification is referred to by nurses for this phase.
4. Implementing Phase- In this phase the decided plan of action is followed by the nurse. The plan of action is specific for each patient in accordance to the diagnosis.
5. Evaluation Phase- Once all nursing intervention actions are over evaluation is done by the nurse to determine which goals have been met. In case there is no improvement in the patient's condition the whole process is carried out again from the first step.
Direct caring includes performing medical tasks for the patient, taking care of the patient's health and indirect caring includes keeping the emergency cart under check or educating the patient about the outcomes of the treatment.
